ABSTRACT

Abstract Objectives: Laryngomalacia is the most common congenital cause of stridor; the natural history of the disease runs through to complete resolution by the age of two. Severe cases are characterized by cyanosis, hypoxia, apnea, furcular and/or subcostal retractions, aspirations, pulmonary hypertension, and failure to thrive and must undergo surgery. This study aimed to evaluate the success rates of supraglottoplasty in our hospital and evaluate the predictive factors for surgical success. Methods: Cohort study, prospectively planned. 75 patients undergoing endoscopic surgery from July 2007 to July 2016 were analyzed at the Santo Antônio Children's Hospital. The primary outcome was percentage of surgical success, defined as the absence of respiratory symptoms or presence of a mild stridor without retractions on the first post-operative month (late success). The secondary outcomes were the early surgical success (absence of respiratory symptoms or presence of a mild stridor without retractions on the first post-operative day). Results: 39 (58.2%) were male, with an average of 4.9 months. Surgical success on the first day was 80.6% (n = 54). At the end of the 1st month, surgical success was 88.6%, considering only those who completed assessment. Twenty-one (34%) presented comorbidities. Presence of comorbidities, pharyngomalacia and GERD were associated with a worse result on the 1st postoperative day, whereas, at the end of the first month, presence of comorbidities, concomitant injuries (tracheo and bronchomalacia) and pharyngomalacia were the predictive variables of surgery failure. Conclusion: Supraglottoplasty has high rates of efficacy and low morbidity. The presence of comorbidities and pharyngomalacia has shown association with a worse early and late surgical outcome. Synchronous airway lesions predict a worse surgical result at the end of the first month. GERD was associated with obstructive symptomatology only in the 1st post-operative day. Abstract Introduction: Chronic rhinosinusitis with nasal polyps, a prevalent disease affecting around 2% of the world population, is characterized by symptomatic inflammation of the nasal mucosa and impairment of quality of life. Chronic rhinosinusitis with nasal polyps has a multifactorial etiology, involving a dysfunctional host response to environmental factors. Thus, inflammatory models may be useful to shed light on the pathophysiology of this disease. Micronucleus count has been used to screen DNA damage in various tissues. Objective: To investigate the association between frequency of micronucleus in exfoliated cells from the nasal cavity of patients with chronic rhinosinusitis with nasal polyps and disease severity. Methods: This cross-sectional study included 21 patients with chronic rhinosinusitis with nasal polyps and 19 controls without disease. None of the participants were smokers. Results: Mean micronucleus count was 3.690 per 1000 cells (±2.165) in individuals with vs. 1.237 per 1000 cells (±0.806) in controls; (Student's t test = 4.653, p< 0.001). Nasal surgery in the past 5 years and aspirin-exacerbated respiratory disease were not associated with nicronucleus count (p= 0.251). Conclusion: Micronucleus count seems to be linked to chronic rhinosinusitis with nasal polyps, providing a new perspective for the evaluation of this disorder.


Resumo Introdução: A rinossinusite crônica com pólipos nasais, doença prevalente que afeta cerca de 2% da população mundial, é caracterizada por inflamação sintomática da mucosa nasal e comprometimento da qualidade de vida. A rinossinusite crônica com pólipos nasais tem etiologia multifatorial, envolvendo resposta disfuncional do hospedeiro a fatores ambientais. Assim, modelos inflamatórios podem ser úteis para esclarecer a fisiopatologia dessa doença. A contagem de micronúcleos tem sido usada para rastrear danos no DNA em vários tecidos. Objetivo: Investigar a associação entre a frequência de micronúcleos em células esfoliadas da cavidade nasal de pacientes com rinossinusite crônica com pólipos nasais e a gravidade da doença. Método: Estudo transversal que incluiu 21 pacientes com rinossinusite crônica com pólipos nasais e 19 controles sem doença. Nenhum dos participantes era fumante. Resultados: A contagem média de micronúcleos foi de 3,690 por 1.000 células (± 2,165) nos indivíduos doentes e 1,237 por 1.000 células (± 0,806) nos controles (teste t de Student = 4,653; p < 0,001). A cirurgia nasal nos últimos 5 anos e a doença respiratória exacerbada por aspirina não foram associadas à contagem de micronúcleos (p = 0,251). Conclusão: A contagem de micronúcleos parece estar ligada à rinossinusite crônica com pólipos nasais, proporcionando uma nova perspectiva para a avaliação dessa doença.

RESUMO Introdução Os operadores de call centers são profissionais da voz com alta demanda vocal e, consequentemente, sujeitos a distúrbios e sintomas vocais. Objetivo: Caracterizar aspectos vocais e laríngeos em operadores de um call center que segue as normas regulamentadoras de ergonomia, mensurando autopercepção vocal, avaliação otorrinolaringológica e análise perceptivo-auditiva vocal. Métodos Participaram do estudo 30 operadores de call center, entre 18 e 41 anos de idade. Todos os sujeitos foram avaliados por meio da Escala de Sintomas Vocais - versão brasileira da Voice Symptoms Scale (VoiSS), avaliação otorrinolaringológica por meio de videonasofibrolaringoscopia e avaliação perceptivo-auditiva da voz com a escala GRBASI, que avalia grau de alteração vocal (G), rugosidade da voz (R), soprosidade (B), astenia (A), tensão (S) e instabilidade (I). Resultados Na Escala de Sintomas Vocais, houve correlação entre o domínio geral e os demais subdomínios e entre os subdomínios limitação e físico. Na avaliação otorrinolaringológica, 12 operadores apresentaram alguma alteração, como acúmulo de secreção à fonação e fendas. A análise perceptivo-auditiva da voz encontrou indivíduos com vozes normais e com grau de alteração discreto a moderado, além de correlação entre o item instabilidade da escala GRBASI com o grau geral, rugosidade, soprosidade e astenia. Conclusão Ambientes de trabalho adequados e que primam por cuidados com a voz podem melhorar a qualidade laboral dos profissionais de call center, amenizando os riscos de desenvolvimento de distúrbios vocais. Contudo, fatores físicos, sociais, ambientais, organizacionais e psicológicos podem ocasionar sintomas vocais nesses profissionais da voz.


ABSTRACT Introduction Call center operators are voice professionals with great vocal demand and, consequently, are exposed to voice disorders. Purpose Characterize vocal and laryngeal aspects among operators of a call center that follows the regulatory standards in ergonomics by measuring voice self-perception, otorhinolaryngological evaluation, and voice perceptual-auditory analysis. Methods The study assessed 30 call center operators between 18 and 41 years old. All subjects were assessed using the Escala de Sintomas Vocais (ESV) – the Brazilian version of the Voice Symptom Scale (VoiSS) –, otorhinolaryngological evaluation using fiberoptic video rhinolaryngoscopy, and voice perceptual-auditory assessment using the GRBASI scale that evaluates degree of voice alteration (G), voice roughness (R), breathiness (B), asthenia (A), strain (S), and instability (I). Results The ESV showed a correlation between the overall domain and the other subdomains and between the subdomains limitation and physical. The otorhinolaryngological evaluation revealed that 12 operators had some alteration such as secretion accumulation during phonation and glottic chinks. The voice perceptual-auditory analysis found individuals with normal voice and with slight to moderate degree of alteration, besides a correlation between the item instability in the GRBASI scale and the overall degree, roughness, breathiness, and asthenia. Conclusion Proper work environments that provide care with voice may improve labor quality of call center professionals, thus mitigating the risks of developing voice disorders. Nonetheless, physical, social, environmental, organizational, and psychological factors may cause voice symptoms among those voice professionals.

ABSTRACT

Laringomalácia é o colapso supraglótico durante a inspiração; é responsável por até 75% dos casos de estridor em crianças de até 30 meses. O achado característico é estridor inspiratório; até 20% dos pacientes apresentam quadros graves, sendo necessária intervenção cirúrgica. A supraglotoplastia é o procedimento de escolha e se considera a presença de comorbidades como o maior fator prognóstico para o sucesso da cirurgia. OBJETIVOS: Descrever a casuística de hospital pediátrico terciário, índices de sucesso e analisando fatores prognósticos cirúrgicos. MÉTODO: Estudo de coorte retrospectivo. Incluídos 20 pacientes submetidos à supraglotoplastia no período de julho de 2007 a maio de 2011. RESULTADOS: Dos 20 pacientes, 13 (65%) eram do sexo masculino; a média de idade no procedimento foi de 6,32 meses. Na endoscopia, 12 apresentavam associações de tipos de laringomalácia, 40% apresentavam faringomalácia associada e três apresentavam também traqueomalácia. Treze crianças apresentavam laringomalácia isolada e sete pacientes apresentavam doença do refluxo gastroesofágico. Quinze (75%) pacientes foram submetidos à ressecção de pregas ariepiglóticas. Após o procedimento, 11 pacientes ficaram assintomáticos e dois necessitaram traqueostomia. A presença de comorbidades foi a variável preditora de desfecho desfavorável pós-operatório (p = 0,034). CONCLUSÃO: A supraglotoplastia é um procedimento seguro para tratamento de laringomalácia em casos selecionados.


Laryngomalacia is the condition responsible for 75% of the cases of stridor in children aged up to 30 months, in which there is supraglottic collapse during inhalation. Inspiratory stridor is a characteristic telltale. As many as 20% of the patients are severely affected and require surgery. Supraglottoplasty is the procedure of choice and the presence of comorbidities is the most relevant prognostic factor for surgery success. OBJECTIVE: To describe a series in a tertiary pediatric hospital, its success rates, and surgery prognostic factors. METHOD: This retrospective cohort study enrolled 20 patients submitted to supraglottoplasty between July 2007 and May 2011. RESULTS: Thirteen (65%) patients were males; mean age at the time of the procedure was 6.32 months. Endoscopic examination showed that 12 subjects had combined forms of laryngomalacia, 40% had associated pharyngomalacia, and three also had tracheomalacia. Thirteen subjects had isolated laryngomalacia and seven had gastroesophageal reflux disease. Fifteen (75%) patients underwent aryepiglottic fold resection. After the procedure, eleven patients were asymptomatic and two required tracheostomy. Presence of comorbidities was the strongest predictor of unfavorable postoperative outcome (p = 0.034). CONCLUSION: Supraglottoplasty is a safe therapeutical procedure for select patients with laryngomalacia.
